Source: Radiology<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p style=\"text-align: justify;\">Combined analysis of 7T R2* and phase data may be able to distinguish  between changes in tissue myelin and iron in multiple sclerosis (MS)  patients, and could help in characterizing the disease process of MS  lesions, according to a study published in the Nov. 14 issue of <em>Radiology<\/em>.<\/p>\n<p>Jeff H. Duyn, PhD, of the National Institutes of Health in Bethesda,  Md., and colleagues wanted to test the effectiveness of recently  available ultra-high-field-strength MRI systems in characterizing MS.  Specifically, they wanted to see if the improved contrast of  susceptibility-weighted 7T MRI images, which is highly variable within  and among MS lesions, is correlated to levels of iron and myelin.<\/p>\n<p>The researchers obtained high-spatial-resolution  susceptibility-weighted 7T MR images from 21 MS patients and  investigated contrast patterns in quantitative phase and R2* images in  areas defined as chronic MS lesions. Positive or negative phase shifts  were assessed in each lesion. Duyn et al also performed postmortem MR  imaging at 7T and 11.7T and the results were correlated with  immunohistochemical staining for myelin, iron and ferritin.<\/p>\n<p>Results showed that 60.5 percent of the lesions had a normal phase and a  reduced R2*, while 38.2 percent had negative phase shift and a variety  of appearance on R2* maps. When compared with histologic findings, R2*  reduction corresponded to severe loss of iron and myelin, while negative  phase shift corresponded to focal iron deposits with myelin loss.<\/p>\n<p>\u201cChanges in these parameters are interpreted as indicators of myelin  and axon loss, which may follow the initial inflammatory process in  MS-induced [white matter] lesions,\u201d wrote the authors.<\/p>\n<p>MS is a  demyelinating disease, though prior to the current study, it was not  known the extent to which myelin and iron contributed to contrast  variations in susceptibility-weighted MR images.<\/p>\n<p>\u201cThe observed  R2* decreases suggest profound myelin loss, whereas negative phase  shifts suggest a focal iron accumulation,\u201d concluded the authors. \u201cThe  tissue concentration of nonheme iron and myelin are particularly  relevant to MS as they may be reflective of the underlying disease  process.\u201d<\/p>\n<p>The researchers noted that one of the limitations of  the study was that only one postmortem brain was available for  investigation. Spostamenti di fase positiva o negativa sono stati valutati in ogni lesione. Duyn  et al post-mortem anche eseguito la RM a 7T e 11.7T ei risultati sono  stati correlati con colorazione immunoistochimica per la mielina, il  ferro e la ferritina.<\/p>\n<p>I  risultati hanno mostrato che il 60,5 per cento delle lesioni avuto una  fase normale e un * R2 ridotto, mentre il 38,2 per cento aveva  spostamento di fase negativa e una variet\u00e0 di comparsa sulle mappe * R2.  Quando  confrontato con i risultati istologici, R2 * riduzione corrispondeva a  grave perdita di ferro e di mielina, mentre sfasamento negativo  corrisponde a depositi di ferro focale con perdita di mielina.<\/p>\n<p>&#8220;I  cambiamenti in questi parametri sono interpretati come indicatori di  perdita di mielina e assoni, che possono seguire il processo  infiammatorio iniziale in modalit\u00e0 MS-indotta [sostanza bianca]  lesioni&#8221;, ha scritto gli autori.<\/p>\n<p>La  sclerosi multipla \u00e8 una malattia demielinizzante, anche se prima di  questo studio, non si sapeva fino a che punto della mielina e il ferro  ha contribuito alle variazioni di contrasto nella suscettibilit\u00e0  immagini pesate MR.<\/p>\n<p>&#8220;La  diminuzione osservata * R2 suggeriscono una profonda perdita di  mielina, mentre si sposta fase negativa suggerire un accumulo focale di  ferro&#8221;, ha concluso gli autori. &#8220;La  concentrazione tissutale di ferro non-eme e di mielina sono  particolarmente rilevanti a MS in quanto potrebbero essere riflessivo  del processo di malattia di base.&#8221;<\/p>\n<p>I  ricercatori hanno notato che uno dei limiti dello studio \u00e8 stato che  solo un cervello post-mortem era disponibile per le indagini. Duyn  et autres ont \u00e9galement effectu\u00e9 l&#8217;autopsie IRM 7T et \u00e0 11,7 T et les  r\u00e9sultats ont \u00e9t\u00e9 corr\u00e9l\u00e9s avec coloration immunohistochimique pour la  my\u00e9line, le fer et la ferritine.<\/p>\n<p>Les  r\u00e9sultats ont montr\u00e9 que 60,5 pour cent des l\u00e9sions avaient une phase  normale et une R2 * r\u00e9duite, tandis que 38,2 pour cent avaient d\u00e9phasage  n\u00e9gatif et une vari\u00e9t\u00e9 d&#8217;apparition sur les cartes * R2. En  comparaison avec les r\u00e9sultats histologiques, R2 * de r\u00e9duction  correspondait \u00e0 une perte importante de fer et de la my\u00e9line, tandis  d\u00e9phasage n\u00e9gatif correspondait \u00e0 des gisements de fer focal avec la  perte de my\u00e9line.<\/p>\n<p>\u00abLes  variations de ces param\u00e8tres sont interpr\u00e9t\u00e9s comme des indicateurs de  la my\u00e9line et la perte axonale, qui peut suivre le processus  inflammatoire initiale dans MS-induite [substance blanche] des l\u00e9sions&#8221;,  \u00e9crivent les auteurs.<\/p>\n<p>La  SEP est une maladie d\u00e9my\u00e9linisante, bien avant l&#8217;\u00e9tude actuelle, on ne  savait pas dans quelle mesure la my\u00e9line et de fer ont contribu\u00e9 \u00e0 des  variations dans la sensibilit\u00e9 de contraste des images pond\u00e9r\u00e9es MR.<\/p>\n<p>\u00abLes  diminutions observ\u00e9es * R2 sugg\u00e8rent une profonde perte de my\u00e9line,  tandis que les d\u00e9calages de phase n\u00e9gative sugg\u00e8re une accumulation de  fer central\u00bb, a conclu les auteurs. &#8220;La  concentration tissulaire du fer non h\u00e9minique et de la my\u00e9line sont  particuli\u00e8rement pertinentes pour MS, car ils peuvent \u00eatre le reflet de  l&#8217;\u00e9volution de la maladie sous-jacente.&#8221;<\/p>\n<p>Les  chercheurs ont not\u00e9 que l&#8217;une des limites de l&#8217;\u00e9tude \u00e9tait que seul un  cerveau post-mortem \u00e9tait disponible aux fins d&#8217;enqu\u00eate.
